I've suffered from migraines since I was about 12. I've tried everything. On good days, I can abort most attacks with paracetamol (acetaminophen) and aspirin. Bad days require Imitrex, and really bad attacks require opiates.

I was 22 and volunteering for a year in Kenya. One weekend, I was about 3 days into one of the worst headaches of my life. Paracetamol and tramadol didn't touch it, and in desperation I went to the A&E (emergency) department of a private hospital. The doctor was unfamiliar with migraine and after a quick review of his textbook he wrote for 100mg IM of pethidine (Demerol), 10mg of Reglan, and 10 mg of diazepam...

He told me to take the Rx to the nurse's station and have them administer the shots. The nurses station left something to be desired. There were used syringes sitting in a waste barrel and no cover on the exam table. I told the nurse my friend was a veterinarian and she could give me the shots at home. The nurse bought it and I was on my way.

I got back to my apartment and read the vial of pethidine. It was labeled for SQ, IM, or IV use. In for a penny in for a pound. Took a fresh needle and administer it into my rt AC vein. OH MY GOD. It felt like the bottom dropped out and the rush was unimaginable. It lasted about 10 min and all was back to normal.

Now in Africa, its common for the pharmacist to give you back the Rx. So a few days later I went to another chemist and picked up a few more ampoules. I experimented with various doses (up to 300mg w and wo diazepam). Don't get me wrong. The rush was amazing. But others are right. It did change my chemistry and nothing compared to that first hit.

Fortunately I'm back in the US where this is much harder to come by. If not, I can't imagine what kind of addicted mess I'd be.
