Abstract
Introduction D-lysergic acid diethylamide tartrate, was first synthesized by Hoffman in 1938. It is a hallucinogenic drug. The user may continue to halluciante, or exhibit paranoid symptoms, delusions or anxiety. Bad "trips" on L.S.D. have lasted as long as four months. Fisher claims that "it.is possible to have recurrences of the original symptoms up to three years after taking L.S.D., without using the drug again." Many stimuli can cause recurrences of the original trip, such as intense emotion, certain kinds of music and intense lights. Some patients report from fifteen to a hundred flashbacks per day. Some individuals manifest long term changes after using L.S.D. only once or twice. Case Report: A 28 year old white male presented for routine dentoalveolar surgery. A complete medical history and examination revealed a well developed, well nourished man admitting no previous drug abuse. Over a 30 minute period the patient received diazepam 15 mg., meperidine 50 mg., methohexital 75 mg. and atropine 0.4 mg., supplemented with 3 liters of 02 and 3 liters of N20 delivered via nasal mask. Local anesthetic consisting of 2 0docaine with 1:100,000 epinephrine was used for block and infiltration. Three impacted third molars were surgically removed, sponge packs were removed and anesthesia was terminated uneventfully. Vital signs remained stable with blood pressure at 120/70, pulse at 67/min and respirations 25/min. Fifteen minutes after termination of the procedure the patient appeared cataleptic, with eyes opened, staring straight ahead. He failed to respond to moderately painful stimuli or verbal commands. Fifty minutes later the patient was able to relate his experience of flashbacks. At this time, the patient revealed a history of L.S.D. use terminating ten years previously. Conclusion: With significant numbers of current and past drug users being treated by practicioners, the aforementioned experience is not unique. Fortunately, there were no residual effects, and the patient reported no further occurrences of hallucinations during the follow-up period.
